The following required documents must be submitted to complete your enrollment:

If you are single:
- Two (2) Forms of ID (At Least ONE Federally-Approved Document)
- One (1) paystub from current employer -OR- a copy of your recent tax return
If you are married:
- We will contact you if we need your spouse’s information to complete the processing of your program.
If you own you own business or are self-employed:
- Two (2) forms of ID (At Least ONE Federally-Approved Document)
- Statement of Income letter on company letterhead
