Step 1 is Take the Forgiveness Survey to see if you qualify.
You will receive an email with survey results andĀ enrollment documents that will allow you to upload in a secured environment the documents we will need to process your enrollment.
Documents vary depending on your personal situation and survey results.
Here are the different documents required, depending on your situation:
If you are single:
* Two (2) forms of ID (federal-approved)
* One (1) paystub from current employer or 2 months of banks statements*
* First two (2) pages of last year’s tax return (if available)
If you are married:
* Two (2) forms of ID (federal-approved) for person with debt
* First two (2) pages of last year’s joint tax return
– OR-
Ā * Paystub(s) or 2 months of banks statements for both you and your Spouse*
If you own you own business or are self-employed:
* Two (2) forms of ID (federal-approved)
* Statement of Income letter on company letterhead
